PLAYER REQUEST

When registering a player for one of our Recreational Programs, a parent/guardian may request that their child be placed on a team with another child. Please complete the Player Request Form and we’ll get back to you shortly.

Each player may make only one request and it must be reciprocated by the player/parent named on the request form. Both players' information and parent/guardian signatures must be on the Player Request Form. Once a player is included on a request form, this is considered their one request and no other requests submitted naming the same player will be considered.

All player request forms need be emailed to the House League Administrator, Gordon: Gordon@markhamsoccer.org before the deadline date.

*ALL PLAYER REQUESTS MUST BE MADE ON OR BEFORE THE DEADLINE. PLAYER REQUESTS RECEIVED AFTER THE DEADLINE DATE, ARE NOT GUARANTEED.
