REQUEST YOUR PLAYER TO JOIN US!
When registering a player for our Recreational Program, a parent/guardian may request that their child be placed on a team with another child. Please complete the Player Request form and we’ll get back to you shortly.
Each player may make only one request and it must be reciprocated by the player/parent named on the request form. Both players' information and parent/guardian signature must be on the Player Request Form. Once a player is included on a request form, this is considered their one request and no other requests submitted naming the same player will be considered.
All requests must be made on or before the deadline. Absolutely no requests will be granted after that date.
